*PFH Coalition Member Application: This PFH Coalition Member application registers your business as a PFH Coalition Member. Please complete this form to start your membership. Upon completing this form, turn it in to Kelly Tsutsui or submit by fax to 808-638-0757.
*PFH Re-order Form: Please use this form to order your PFH bags and PFH bottles when to be sold at your business location.
*PFH Star Levels for Businesses: This checklist ranks the level your business will qualify for upon registration. Also on this sheet is a list of benefits your business will receive at each start level category.
